We are accepting new referrals. A referral is not required, but the information you provide will help us serve your patient better.
The typical wait time for a first appointment is 1 to 2 weeks; we see urgent patients more quickly. We are working to offer telehealth appointments and limited in-person appointments during the spread of COVID-19.
Our subspecialty clinic is staffed by pediatric and adolescent gynecology surgeons. If your patient has a nonsurgical need, we may redirect your referral to Reproductive and Sexual Health (another subspecialty clinic in Adolescent Medicine), which is staffed by adolescent medicine physicians and advanced practice providers (APPS). This will allow your patient to receive care more quickly.
Referral requirements
When submitting a referral, please ensure that it is complete. This helps us schedule your patient’s appointment in a timely manner and ensure their first visit is smooth and productive.
Please include:
- Service/specialty clinic requested
- Reason for referral: what is the clinical question for the specialist?
- If the referral is urgent (see within 4 weeks) or routine (next available)
- ICD-10 diagnosis – required
- Visit type:
- New patient consult, transfer of care, second opinion or return visit/ongoing care
- ALL relevant clinical documents
- Most recent heart rate and blood pressure
- Clinic notes
- Medication history
- Growth charts/curves
- Lab reports
- Diagnostic reports (e.g., ultrasound, MRI, CT) and imaging
- Previous specialty evaluations
- Patient’s full name, DOB, sex, address, guardian contact information and insurance
- Referring provider’s name, phone, fax and the referral coordinator’s email address so that we may contact you if additional information is needed
- Preferred clinic location
- If an interpreter is needed
- Any known barriers to performing a successful telehealth (video) visit with the family

What Your Patients Can Expect
- A family’s first appointment will include meetings with more than one provider and a social worker when appropriate.
- First appointments typically last at least 2 hours.
- At the first appointment, we meet with the caregiver and patient together and meet separately with the patient alone if they are over the age of 12.
- Depending on the concern, a physical exam, including a pelvic exam, may be indicated.
- At times we have trainees in the clinic, including students, residents and fellows.
