Before your appointment
- We will provide forms for you to fill out.
- Before your child’s appointment, please complete the Rehabilitation Psychology Questionnaire and the request for your child’s school records. Your child must sign the request for school records if they are 13 years old or older.
- Mail the questionnaire and school records in the prepaid envelope we provide or email the forms to [email protected].
- Gather your child’s school testing records, Individualized Education Program (IEP), 504 plans and report cards.
- Collect other helpful information like reports of previous neuropsychological or speech-language evaluations.
- Learn more about what to do before your appointment.
